*JOB DESCRIPTION:*
Medical Scribes are trained medical professionals responsible for accurate documentation of a patient’s medical information.
The primary goal of the scribe’s position is to increase the efficiency and productivity of the clinician.
It will be the responsibility of the scribe to annotate any dictated or written information for patient treatment plans into the electronic medical record.
The Scribe is also responsible for following all state and federal guidelines for proper documentation.
This includes but is not limited to following HIPAA guidelines.
It is the scribes’ duty to clearly record each patient’s history, exam notes and clinician’s plan of care along with prescriptions provided and diagnosis given.
*Job Duties*
· Assist clinician with documentation of each patient’s medical chart during the visit.
· Accompany clinician into the patient exam to transcribe the history and physical examination of each patient as given by the clinician.
· Document procedures performed by clinician with total accuracy and detail.
· Transcribe any consultations or discussions with family members and/or clinician.
· Order in-house testing for labs, prescriptions, referral follow-ups and other diagnostic testing ordered.
· List all proper diagnoses as well as the applicable ICD-10 coding and a complete health history.
*Knowledge and skillset requirements*
· Strong typing skills are REQUIRED (50 WPM), proper spelling, and grammar are also a requirement of this position.
· A strong knowledge of medical terminology and diagnosis coding.
· Impeccable communication skills
· Ability to observe in a professional, non-disruptive manner while transcribing.
· Strong attention to details
· Knowledge of computer and phone systems (Microsoft word, teams, excel and outlook)
· Ability to maintain professionalism in all encounters with staff and patients.
· Understanding of clinic flow, the scribe needs to have the ability to maintain efficient exam times to support the clinician in staying on time with patient appointments.
