* The Licensed Vocational Nurse ensures quality and safe delivery of health care services in home and/or facility, patient plan of treatment, and agency policies and procedures under the supervision of a registered nurse / Clinical Supervisor or Associate Director of Patient Care.
* Re-evaluates the patient’s nursing needs q visit/shift, updating the physician orders, medication profile, MAR & POC as required.
* Initiate appropriate preventive and rehabilitative nursing procedures.
* Counsel and instruct the patient, family, and other nursing personnel; follow all regulation guidelines, i.
e.
; mandatory reporting, Title 22, and Medicare CoP’s.
Medi-Cal’s directives.
* Provides direct patient care, evaluates outcomes, consult with other practitioners as required and adjusts nursing care processes as appropriate to ensure optimal patient care under direction from the RN.
* Advocate for patient rights and identify potential conflict.
Administer medications and treatments as prescribed and LVN II performs venipuncture for lab draws, reports results to MD and initiate new orders if indicated by MD.
