Job Title: Registered Nurse (RN) - Nursing Home Long Term Care (LTC)
Job Summary:
The Registered Nurse (RN) in Nursing Home Long Term Care (LTC) is responsible for coordinating and providing skilled nursing care to residents in a long-term care facility.
The RN will collaborate with an interdisciplinary team to implement individualized care plans, administer medications, monitor vital signs, and ensure the overall well-being of residents.
Responsibilities and Duties:
1.
Assess the physical, social, and emotional needs of residents and develop comprehensive care plans.
2.
Administer medications and treatments prescribed by healthcare providers.
3.
Monitor and document residents' vital signs, changes in condition, and response to interventions.
4.
Collaborate with other healthcare professionals, including doctors, therapists, and social workers, to ensure continuity of care.
5.
Provide emotional support to residents and their families, fostering a compassionate and caring environment.
6.
Assist with activities of daily living, including bathing, dressing, and mobility, as needed.
7.
Implement infection control practices and maintain a safe and clean environment for residents.
8.
Advocates for resident rights and ensures their autonomy, privacy, and dignity at all times.
9.
Educate residents and their families on health conditions, medications, and self-care techniques.
10.
Participate in interdisciplinary team meetings to discuss resident progress, care plans, and discharge plans.
11.
Maintain accurate and up-to-date medical records and nursing documentation.
12.
Stay updated with nursing best practices, regulations, and policies relevant to long-term care settings.
13.
Encourage and promote an atmosphere of teamwork among nursing staff and other healthcare professionals.
14.
Demonstrate professional and ethical behavior in all interactions with residents, families, and coworkers.
15.
Adhere to all federal, state, and facility-specific guidelines and regulations.
Qualifications and Skills:
1.
Valid and unrestricted Registered Nurse (RN) license in the relevant state.
2.
Graduation from an accredited nursing program.
3.
Prior experience in long-term care or geriatric nursing preferred.
4.
Sound knowledge of nursing theories, practices, and techniques related to long-term care.
5.
Strong assessment and critical thinking skills to respond effectively to emergency situations.
6.
Excellent interpersonal and communication skills to interact with residents, families, and healthcare team members.
7.
Ability to prioritize tasks, manage time effectively, and multitask in a fast-paced environment.
8.
Proficiency in using electronic medical record (EMR) systems and other relevant computer applications.
9.
Willingness to work flexible hours, including weekends and holidays, as needed.
10.
CPR certification and knowledge of basic life support (BLS) protocols.
11.
Demonstrated commitment to continuous learning and professional development.
