JOB SUMMARY
Under the supervision of the Supportive Services Center Manager, the CalAIM Referral Coordinator will work with case management/clinical teams, community service providers and patients to ensure the timely process of Enhanced Care Management (ECM) and Community Support referrals.
These programs work with individuals who are homeless and have other physical or mental health conditions.
This will be accomplished by reviewing project guidelines for the programs, contacting appropriate patients, staff and outside agencies as needed, and tracking these results on an individual and group basis with the electronic referral system.
JOB DUTIES AND RESPONSIBILITIES
Receives and processes referrals in a timely manner.
Revising referrals when required based on availability and resources.
Communicate daily with Community Providers, County, Health and Human Services Representatives and managed care as needed.
Coordinate documentation and tracking within the guidelines of CalAIM (ECM and CS).
Review and upload required reports to managed care monthly.
Responds promptly to inquiries as to status of placement by assessing the request and evaluating the circumstances to provide the correct information.
Initiates the appropriate documentation of requests from appropriate agencies.
Follows-up on missing documentation to obtain authorization or scheduling when appropriate.
Record status of referrals.
Uses various modules of the computer system to obtain information, schedule appointments, query information and obtain authorizations.
Indexes incoming reports pertaining to referrals for assigned agencies.
As needed, support ECM and CS case managers with field visits.

Ability to exercise discretion and independent judgment in a variety of decision making scenarios
Good attention to detail and ability to probe for information to fully understand the needs of the customer
Strong organizational and prioritizing skills
Ability to work within a team and to manage conflict
Excellent written and verbal communication skills
Use of Partnership and other insurance websites for processing authorizations and verifying eligibility.
EDUCATION & EXPERIENCE
Experience with Microsoft Office programs (Word, Excel, Power Point)
Experience creating reports for grant related work
PREFERRED QUALIFICATIONS
Minimum Associates degree in Health Information, Business, or related field.
Minimum one year experience working in a social service or medical field setting.
Minimum one year experience working with at risk populations.
