Job Title: Behavioral Health Consultant
Department: Medical
FLSA Status: Non-Exempt
Reports To: Integrated Operations Director
Hours: Monday - Friday 8:30am - 5:00pm
Location: Center of Hope Clinic in Redding and at least 1-2 days at our Round Mountain Clinic
POSITION DESCRIPTION:
The Behavioral Health Consultant works collaboratively within the care team to provide services to patients with mental health, substance use and other chronic health conditions.
The position necessitates physical presence onsite to support a sense of stability and predictability for day-to-day problem solving with both staff and patients.
POSITION SUMMARY: Under limited supervision, counsels' individuals, families or groups regarding psychological or emotional problems such as stress, substance abuse, or family situations and develops and implements a therapeutic treatment plan within the primary care setting.
Works well independently and as a part of the integrated care teams to maintain continuity of care.
ESSENTIAL DUTIES AND RESPONSIBILITIES: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
Other duties may be assigned which may become essential to the position.
General bio psychosocial assessment with comprehensive treatment plan recommendations Be responsive to medical providers and care team for warm hand-offs to address any behavioral health needs presented on the medical floor Special consult (for psychiatry visits or chronic pain risk evaluation) with patients upon request from medical providers Acute needs/risk assessment Addiction screening and referral Provide evidenced-based interventions and psychotherapy services to patients with chronic mental health and functional impairment concerns Assists patients in identifying and tracking goals for healthy living and documents these self-management goals in chart Available to diffuse emotional crises and assist in immediate needs and risk assessments Works in collaboration with primary care, behavioral health, substance use teams and other programs as a part of the integrated care team Identifies and reaches out to patients in need of referral to in-house counseling services, psychiatry services to address behavioral health needs Keep track of appointments in Outlook calendar so that team knows when BHC will be out of the office SUPERVISORY RESPONSIBILITIES: This job has no supervisory responsibilities.

EDUCATION/EXPERIENCE: Current LCSW or LMFT License or Registered Associate working toward licensure (ACSW or AMFT).
Experience in rural community preferred.
CERTIFICATES/LICENSES: California LCSW or LMFT License or Registered Associate.
Current CPR certification.
Current drivers’ license and insurance.
LANGUAGE ABILITY: Must have intermediate level language skills.
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
Ability to write routine reports and correspondence.
Ability to speak effectively before individuals, groups or employees of organization.
MATH ABILITY: Must have basic math skills.
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
Ability to compute rate, ratio, and percentages.
REASONING ABILITY: Must have very high level reasoning skills.
Ability to define problems, collect data, establish facts, and draw valid conclusions.
Ability to interpret an extensive variety of technical information and deal with several abstract and concrete variables.
COMPUTER SKILLS: To perform this job successfully, an individual should have knowledge basic working knowledge of Windows operating systems, Microsoft Office Suite - Word, Excel, Outlook, and Internet browsers.
Must have working experience with medical management software such as Health Pro.
EQUIPMENT SKILLS: Proficient with computers, printers, copiers, multi-line telephone systems.
