JOB SUMMARY
The Enhanced Patient Services Care Coordinator will work in a team environment to assist individuals at risk of homelessness or who are homeless and are high utilizing Medi-Cal and Medi-Medi beneficiaries in an intensive case management model.
The Care Coordinator will provide support for intensive medical case management system for people enrolled in intensive case management programs and will support the following activities; screening and enrollment of participants, field visits, assessment, care coordination, connection to medical resources, connection to community resources, education, patient empowerment and utilization review.
JOB DUTIES AND RESPONSIBILITIES
Review daily referrals; meet with clients in order to obtain program consent, administer required assessments, and review client’s care plan, medical follow-up and psychosocial needs.
Work collaboratively with the HOPE Medical Team and Enhanced Services Case Management Team.
Assist client with meeting the goals identified in their care plan.
Assist in timely scheduling of primary care and other medical appointments to support client health.
May attend appointments with clients
Link individuals to the application process for various community assistance programs
Maintain good working relationship with community health partners by working as a liaison with other agencies and the community
Document all clinical support tasks to meet data collection requirements
Complete all necessary billing paperwork to support client services.
Participate in quality assurance activities to monitor and improve client service quality
Review hospital inpatient and ER reports for program participants
Assist in locating clients in support of field visits, as part as a multidisciplinary team
Participate in on-going training and reviews
Must be able to prioritize multiple projects
Other duties as assigned
KNOWLEDGE, SKILLS, AND ABILITIES
Proficiency with Microsoft Office applications including Outlook, Word, Excel and PowerPoint
Ability to prioritize projects, work relatively independently, manage multiple tasks, and meet deadlines
Strong organizational skills
Strong written and verbal communication skills
Ability to effectively present information, respond to questions, and generally communicate clearly and respectfully with participants, coworkers, other agencies and companies
Comfortable discussing sensitive health issues with clients
Strong interpersonal skills.
Ability to work with people with a variety of backgrounds and educational levels both in person and on the phone
Ability to work independently and as part of a team
Good judgment, problem solving and decision-making skills
Ability to uphold client confidentiality with the utmost professionalism
Ability to travel to other offices and community locations
Support patients in a non-judgmental way.
Ability to complete CPR certification
EDUCATION & EXPERIENCE
High School diploma or GED required
Current CA Driver’s License
PREFERRED QUALIFICATIONS
Experience working in a healthcare setting
Prior case management experience
Community Health Worker Certificate (CHW), Medical Assistant or Certified Nursing Assistant Certificate (CNA) with one year of community health experience, or a minimum of 5 years of community health/field work or other relevant experience.
