An Audiologist must have the following credentials:
* Audiologist License
* Curriculum Vitae (Resume)
* ADA/OSHA Compliant – Wheelchair accessible location (Photos required)
* Sound Room Certificate of Measurement (Octave Band CenterFreq Hz.
)
* Insurance Certificate
